Document Checklist for Adjustment of Status for Natives or Cuban Citizens

1. Payment of $1440.00 to "U.S. Department of Homeland Security" with debit or credit card
2. Two (2) passport-size photos (2 inches by 2 inches).
3. Government-issued photo identification.
4. Birth certificate.
5. Evidence of being a native of Cuba or a Cuban citizen.

For example: If you are the primary applicant, you must present evidence of Cuban citizenship or of being a native of Cuba.

Evidence that you are a Cuban Citizen or Native of Cuba (if you were born in Cuba)

If you were born in Cuba, some examples of evidence you can present to show you are a native of Cuba include, but are not limited to:

• A Cuban passport (Passport of the Republic of Cuba), expired or valid, that shows Cuba as the place of birth of the passport holder.
• Your Cuban birth certificate issued by the corresponding civil registry in Cuba.

Evidence of Cuban Citizenship (if you were born outside Cuba)

If you are a Cuban citizen born outside of Cuba, some examples of evidence you can present to show your Cuban citizenship include, but are not limited to:

• A valid Cuban passport (Passport of the Republic of Cuba)
• Certificate of Nationality
• Letter of Citizenship.

6. Evidence that you have been physically present in the United States for at least one year before the date you file your application.
7. Copy of the page of your passport that has the nonimmigrant visa (if applicable).
8. Copy of the page of your passport that has the admission or permission stamp (issued by a US government immigration officer) (if applicable).
9. Copy of Form I-94, Arrival/Departure Record, or a copy of the admission stamp or temporary stay permit from Customs and Border Protection (CBP) on the travel document (if applicable).
10. Medical Examination.
11. Police records and certified court records regarding criminal charges, arrests, or convictions (if applicable).
12. Documentation of previous J-1 or J-2 nonimmigrant status (if applicable), including proof of compliance with or waiver of the foreign residence requirement under INA 212(e) (for more information see Form I-612, Application for Waiver of the Foreign Residence Requirement).
13. If you currently have A, G, or E status, include Form I-508, Application for Waiver of Certain Rights, Privileges, Exemptions and Immunities.
14. Form I-566, Interagency Record of Request - A, G, or NATO Dependent Employment Authorization or Change/Adjustment to/from A, G, or NATO Status (only if you have A, G or NATO nonimmigrant status)
