Requirements for Form N-400

When applying for US citizenship with Form N-400, you must include some supporting documents as evidence that you are eligible to naturalize.

USCIS requires the following documents to demonstrate an applicant's eligibility for naturalization, verify identity, conduct background checks and make the process fair for everyone.

Proof of residence

• Green Card: Provide a photocopy of both sides of your Permanent Resident Card (Form I-551).
• Lost Green Card: If you lose it, include a photocopy of the receipt of your Form I-90 (Application to replace permanent resident card).

Fee payment

• Required fees: Check the current amount of the Form N-400 fees on the USCIS page.
• Payment methods: send a check or money order with your A number on the back or use a credit card (Form G-1450).

Photographs (if you reside outside the US)

• Two identical photographs: include two color photographs following USCIS guidelines (available on their website).
• Photo details: write your name and A number lightly in pencil on the back of each photo.
• Religious head coverings: make sure your face is clearly visible for identification.

Additional supporting documents for Form N-400

Depending on the situation, some applicants must submit additional supporting documents with their N-400:

• If your name is different from the one on your resident card, send documents such as your marriage certificate, divorce decree, or court documents showing the name change.
• If you are applying based on your marriage to a US citizen, you must send the following four documents:
  • Proof that your spouse has been a US citizen for the last three years
  • Proof of your marital status (marriage certificate)
  • Copies of divorce documents, annulment documents, or death certificates if you were previously married.
  • Documents related to you and your spouse (for example, shared tax returns, bank statements, lease agreements, birth certificates of your children)
• Official joint tax returns for the last three years.
• IRS records of your tax returns for the last three years
• If you are in the US military and are applying for citizenship based on your service, include Form N-426.
• If you have been outside the US for 6 months or more, send evidence that you maintained ties to the US, such as tax returns or mortgage payments.
• Form N-648, officially called "Medical Certification for Disability Exceptions", if you are applying for an exemption from the US citizenship test based on a qualifying medical condition.
