Document Checklist for the Work Permit for the First Time and for Renewal

1. Form G-1145 (optional), to receive notices about your application by text message or email.
2. Form I-765
3. A copy of the I-94, Nonimmigrant Arrival-Departure Record
4. A copy of the passport (even if expired).
5. 2 passport-size photographs. Attach the photos to the upper right corner of the first page of Form I-765.
6. A copy of your passport or other government photo identification in English. If the passport or government ID is not in English, include a translation of the identification and a translation certificate.
7. If you are applying to renew a work permit you already have, make sure to check box 1.c. on Form I-765 and include a copy of your current work permit.
